Project Welcome Home

Project Welcome Home (PWH) is the first pay for success project launched in the State of California. The project, implemented in partnership with Abode Services, a leading provider of services for homeless residents in Santa Clara County, serves 150-200 chronically homeless individuals who are also frequent users of the County’s emergency services, mental health facilities and jails.

Palantir, a software company, has configured an integrated data platform to support PWH. The platform has integrated key datasets from different departments in the County to triage and identify the highest need utilizers in the County’s systems, conduct outreach and enrollment in the program and monitor and track client’s progress in the permanent supportive housing program.
