Community Plan to End Homelessness
We are pleased to now present to you the 2020-2025 Community Plan. This is the product of a robust community engagement process led by the Santa Clara County Continuum of Care. For the next five years, this plan will guide the County, cities, nonprofits, and other community members as they make decisions about funding, programs, priorities, and needs.

3-Part Framework
The 2020–2025 Community Plan to End Homelessness will serve as our roadmap for ending homelessness in Santa Clara County and is organized around three core strategies:

Community Engagement
A thorough community engagement process occurred throughout late 2019 / early 2020 and included:
- Subject-matter expert convenings
- Stakeholder focus group meetings
- Key stakeholder interviews
- Consumer focus groups and interviews
- Community meetings
- Survey that yielded more than 5,000 responses
